How to File a Complaint
The list of states below contains direct links to complaint forms for each state’s department of insurance where consumers and policy owners are able to file grievances and complaints against insurance companies and/or agents. Each state handles the processing of complaints differently. Please read your state's information, instructions and processes carefully. Most states allow for the filing of complaints through an online complaint form which, once filed, will be forwarded to the appropriate division within the state’s insurance department. Options to print and mail complaint forms are also available.
Note: "Consumers and policy owners" can be individuals, entities or companies, so long as they are the owner of record.
